This registration form is specifically for 6th graders and older.

NOTE ON SPONSORS
Unlike the Child Baptism form, this version does not include a section for sponsor information. If you would like to have sponsors present and included on the certificates, please use the Child Baptism Form instead.

We offer baptisms DURING our Church services on the 2nd and 4th Sundays of the month. We also offer private baptisms AFTER the 10:30 am service almost every Sunday of the month. ***Please make sure to enter your first and second choice of baptism dates.
