Description

From June 14 through September 6, 2026, FPLC will move to a single summer worship service each Sunday at 9:30 am.

This registration form is specifically for 6th graders and older.

IMPORTANT NOTE ON SPONSORS
Unlike the Child Baptism form, this version does not include a section for sponsor information. If you would like to have sponsors present and included on the certificates, please use the Child Baptism Form instead.
